Improving Care Integration

Delivery of person-centred care depends on having our systems and services more integrated. We do this through tools such as HealthPathways, Joint Planning and delivery of Shared Care programs such as GPCanShare. Primary care continued to play a key role in responding to COVID-19 with CESPHN supporting initiatives to increase vaccination as well as provide additional service capacity.
